- Comprehensive Health Assessments – for patients aged 45–49 and 75+, reviewing your overall health, lifestyle, medications, and risk factors.
- Care Plans (GP Management Plans) – personalised plans developed by your GP to help you access the right allied health and specialist services.
- Regular Reviews – scheduled follow-ups to track your progress and adjust your plan as needed.
- Team Care Arrangements – coordinated care between your GP, nurse, and allied health providers such as dietitians, podiatrists, physiotherapists, and diabetes educators.
- Aboriginal and Torres Strait Islander Health Support – access to annual 715 Health Checks and culturally safe chronic disease care.
